Insurance Information
The initial evaluation visit for head and neck pain is covered by almost all insurance carriers with whom we have filed claims in the past. This includes, but is not limited to, the following health insurance plans: Most insurance companies will pay for a centric relation splint (appliance), if it is preauthorized ahead of time. We have the appropriate authorization forms and will send them to your insurance company with medical necessity documentation to see if they will approve the mouthpiece. If your insurance company approves the splint, you only have to pay the amount specified in your contract, either a set co-pay or a plan percentage and/or deductible. Your insurance will specify the amount for which you are responsible. However, the only way we can know for sure whether your insurance will pay for your splint is to wait until the preauthorization process is completed, and we are notified of the approval or denial of coverage. If you decide to have the splint made before we get the appropriate insurance authorization, you are liable for payment of the splint in the event that your insurance company does not approve the procedure, and we will ask you to sign a statement acknowledging your willingness to pay for services rendered. The Lexington Headache Clinic accepts most of the well-known insurance carriers. We are providers for Anthem Blue Cross/Blue Shield, Blue Grass Family Health, CHA Health, Humana Healthcare, United Healthcare, and Medicare. We do not accept Medicaid or Champus.
